What are we doing?

This survey aims to identify the strengths and resource needs of your community. Your feedback is anonymous. The information you provide from this survey is very important. It will be used to develop the Community Action Partnership service plan to respond to community needs. This community needs assessment survey is being led by the County of San Diego, Health and Human Services Agency (HHSA), Department of Homeless Solutions and Equitable Communities, Office of Equitable Communities (OEqC), and Community Action Partnership (CAP).

What is CAP?

The County of San Diego’s CAP is the region’s designated Community Action Agency, administering federal Community Services Block Grant (CSBG) funds for the County of San Diego. CAP’s mission is to promote pathways for people, local businesses, and communities to overcome barriers to prosper and thrive.

What are we trying to measure and what will be done with the survey information?

This survey is required by the CSBG and is focused on capturing comprehensive data and community input that will inform CAP’s goals, programs, and services provided to individuals, families, and communities. To support the County’s vision for a “just, sustainable, and resilient future for all,” the information collected in this survey centers around the County’s Live Well San Diego top five areas of influence: health, knowledge, standard of living, community, and social. Your responses will allow CAP to identify what current programs and services are successful and what areas need improvement, including barriers to services, gaps in services, and ideas for future programs and services. The information collected by these surveys will not only be utilized to help guide CAP programs and resources but will also be made available to the public and other service providers, including, but not limited to, other County divisions and departments, community organizations, and groups such as resident leaders working in the community, to help them identify opportunities for collaboration to meet community needs.
