Amherst Regional Public Schools: Children of Military Families Survey

Question Title

* In accordance with the Massachusetts Department of Elementary and Secondary Education, Amherst Regional Public Schools is conducting a survey to determine which students are members of a military family as defined by the Interstate Compact on Educational Opportunity for Military Children.

If you are a parent of a student currently attending Amherst Regional Public Schools, please indicate if you are/were any of the following on or before October 1, 2019:

Active duty members of the uniformed services, National Guard and Reserve on active duty orders

Members or veterans who are severely injured and medically discharged or retired within (1) year (medically discharged between 10/2/2018 and 10/1/2019)

Members of the uniformed services who die on active duty or as a result of injuries sustained on active duty for a period of 1 year after death

None of the Above
