AAA Needs Assessment Survey - For Seniors, People with Disabilities and their Caregivers

Sonoma County Residents, We Want to Hear from You!

Your participation in this Community Needs Assessment survey will help the Area Agency on Aging (AAA) plan and develop services to meet the needs of older adults, people with disabilities and their caregivers in Sonoma County.

The survey should take 10 minutes or less to complete and is open to people of all ages. The survey is anonymous and will not affect eligibility to any services you may be receiving. Please complete the survey only once.
