Brand Survey Questions

The California Department of Public Health (CDPH) touches your life every day in many ways, making California communities healthier places in which to live, learn, work, grow and play. We’d like to know how public health touches you and your family. Below is a survey to help us know what you think of public health and how we can be of even greater service to you.

The history of the California public health dates back to the Gold Rush, fighting cholera outbreaks that killed 1,000 Sacramentans in just three weeks. Today, CDPH continues to use science to tackle the emerging public health threats of the 21st Century, to protect the health of every Californian and the millions who visit here every year.
