Requirements and Resources
Please Note:
1. This application/registration is for the COVID-19 vaccine ONLY.

3. Physician clinics may receive Moderna vaccine or Pfizer vaccine. Please note that Pfizer vaccine for 6 months to 4 years old requires reconstitution and appropriate training is required. The training page is here.
4. Supplies (ie. syringes) will be provided by Public Health.
5. Physician clinics who are already receiving COVID vaccine for administration to their patients do not need to reapply. Continue working directly with your local Public Health Office for vaccine.
6. Record the COVID-19 vaccine administered on the line lists available at https://www.ehealthsask.ca/services/Manuals/Pages/COVID-19.aspx and submit daily.
7. If approved, Medical Services Branch will connect your clinic with the SHA local public health office for the purpose of ordering and distributing the COVID-19 vaccine.
