The CP3 Team is rolling out a third round of the SDOH Learning Cohort. Our implementation work will focus on the PRAPARE tool but we will also do several webinars towards the end of the cohort on using data to identify and create needed resources, partnerships and collaborations. Those trainings will not be tied to a particular data collection tool. If you are already using a different tool and want to be included in the later part of the cohort please email me directly so we can keep you in the loop for those resources.

We envision the cohort to be primarily virtual with one in-person meeting, location to be determined based on the distribution of sites in the cohort. We also plan on having coaches and technical assistance available to the sites that are participating in the learning cohort. We need everyone that is interested in this cohort to complete the brief survey by Monday, August 19th by 5pm PT.
