1. CONSENT FORM
The project: SHERLOCK aims to align the identified skills mismatch in the field of energy efficiency evaluation by designing, creating, and piloting a micro-programme based on micro-credentials. In practice, it will provide a technical comprehension of the energy efficiency topic to learners with a SSH (Social Sciences and Humanities) background. Whereas a business background will be provided to learners with a STEM (Science, Technology, Engineering and Mathematics) background.
The survey: We collect information about the current needs and priorities of Vocational Education and Training (VET) students in the energy retrofitting sector to develop the SHERLOCK courses. The data, collected through your participation, will contribute to tailoring the educational materials to the needs of learners. With your express consent, the data will be processed for the purpose of statistical analysis for the course development.
Data Protection. Your answers will be treated confidentially and the information, provide by you, will be kept anonymous in any research outputs/publications. Your data will be processed in accordance with the European General Data Protection Regulation 2016/679 and the Data Protection Act 2018. Your data will be stored securely and will only be viewed by the research team. All data will be deleted after the end of the project by 31.08.2026.

Your rights. You can exercise the following rights:
1. right of access, which means you have the right to obtain from the controller confirmation as to whether or not personal data concerning you are being processed, and, where that is the case, access to the personal data (Art. 15 of GDPR);
2. right to rectification, which means you have the right to obtain from the controller without undue delay the rectification of inaccurate personal data concerning you. Taking into account the purposes of the processing, you have the right to have incomplete personal data completed, including, by means of providing, a supplementary statement. (Art. 16 of GDPR);
3. right to erasure, which means you have the right to obtain from the controller the erasure of personal data concerning you without undue delay and the controller shall have the obligation to erase personal data without undue delay when the request is legitimate (Art. 17 of GDPR);
4. right to restriction of processing, which means you have the right to obtain from the controller restriction of processing when the request is legitimate (Art. 18 of GDPR);
5. right to data portability, which means you have the right to receive the personal data concerning you, which you have provided to a controller, in a structured, commonly used and machine-readable format and have the right to transmit those data to another controller without hindrance from the controller to which the personal data have been provided (Art. 20 of GDPR);
6. right to object, which means you have the right to object, on grounds relating to your particular situation, at any time to processing of personal data concerning you, including profiling based on those provisions. The controller shall no longer process the personal data unless the controller demonstrates compelling legitimate grounds for the processing which override your interests, rights and freedoms or for the establishment, exercise or defence of legal claims. (Art. 21 of GDPR);
7. right to withdraw consent at any time, that means you have the right to withdraw your consent at any time.
