The primary objective of this survey is to collect and share the perspectives of Patient Safety/Medication Safety Professionals on the risk of medication errors resulting from the very small print now commonly found on medication labels, packaging and inserts.

It is the second in a series of surveys, the first of which collected the perspectives of practicing clinicians (primarily nurses and pharmacists) on this issue.
