After completing this form, you will be contacted by a HUD Certified Housing Counseling to complete the intake process.

**After completing the following online survey, you must mail, email, or fax the documents listed below for ALL Household members 18 years or older.**

- Paystubs for ONE (1) month
- Social Security Award Letter (SSI/ SSD/ Retirement)
- Bank Statements ALL accounts for THREE (3) months
- Photo ID (Drivers License, non-Drive ID, Passport, Medicare w/ photo)
- Social Security Card for ALL members of the household
