Views on the support for individuals who are autistic and family/carers

You are being invited to complete this survey as you or someone in your family/who you care for, has had an autism diagnostic assessment and received a diagnosis of autism. 

We are keen to get your views on what good post-diagnostic support would be like. This will help us provide the right kind of NHS support in Tees Valley in future.  

We want to find out about the different kinds of help that people (aged 16 or over) may need following a formal diagnosis of autism.  This service will be for individuals who are not eligible for support under the Care Act.

Listening to your views and experiences 

This is your opportunity to tell commissioners in Tees Valley what future NHS support you would like to have following your autism diagnosis.

Your opinions about what would be helpful are vital in making sure that the right kind of help is offered in future. 

All of the information that is received will be anonymised so that nothing you tell us can be linked back to you.
