CONSENT FORM - PHOENIX LIGHTS NETWORK SURVEY

Explanation of Survey: You are invited to participate in this research as someone who may have an interest in the phenomenon known as the "Phoenix Lights." This study entails completing a confidential and anonymous questionnaire that asks about your experiences related to the Phoenix Lights, as well as other UFO or Unidentified Aerial Phenomena (UAP) activity, and possible contact with nonhuman intelligent beings (NHIBs), such as extraterrestrials, ghosts, or Light Beings.
Purpose and Procedure: The Phoenix Lights Network is conducting this study in order to better identify and understand effects related to sighting of UFOs or Unidentified Aerial Phenomena (UAPs) and/or experiencing contact with nonhuman intelligent beings (people who have these sightings or experience intelligent nonhuman contact are referred to as "experiencers"). The survey questionnaire is completely anonymous and will take about 30 minutes to complete. You will answer questions about sightings, your reactions to those sightings, any contact with nonhuman intelligent beings, and overall outlook on life. We will also ask how an interest in UFOs/UAPs may have changed your beliefs, as well as perceptions of yourself and others. Risks: Although we have made every effort to protect your identity, there is a minimal to nonexistent risk of loss of confidentiality. Our survey platform will not save any identifying information about respondents. Please be forewarned, however, that the questions about the nature of your UFO and UAP-related experiences may elicit anxiety or discomfort for some people.
Benefits: You may or may not benefit from this survey. We will be providing all experiencers, and any people who may help and support them, with results gleaned from this study, via the Phoenix Lights Network website, as well as through professional publications. It is hoped that this information will be helpful to both experiencers and the general public in better understanding these phenomena.
Confidentiality: This is an anonymous survey. We will keep your responses on password-protected computers in password protected files. Once submitted, data collected will not be associated with your name or IP address. Should you choose to pause your completion of the questionnaire and return to it later, you may reuse the same link to pick up where you left off, so long as you are using the same computer and internet browser. Results will be reported in aggregate form only, and individual results will not be analyzed or disseminated, and reports will not include any identifying information tied to specific individuals.
Participation Is Voluntary: Participation in this study is completely voluntary. You may refuse to answer any questions and may withdraw from the study at any time, up to the point when you submit your responses. At that point, there will be no way to identify your information, since all responses are anonymous. There will be no consequences for withdrawal from the study, since there is no way to identify individuals who have participated versus declined to participate.
