The Planning, Institutional Effectiveness, and Research Department (PIR) strives to provide accurate and clearly presented information in a timely fashion. Prior to submitting your request for institutional data, please review the information already available on PIR’s Intranet site. If you are unable to locate the information on PIR’s Intranet site or need additional information, please continue to complete the Data Request Form.

A few notes before completing the form…
  • The typical time to complete data requests is two to three weeks (10 to 15 business days). Please give as much notice as possible when requesting data.
  • If you have submitted a similar data request before, please send what was previously provided by PIR (send via e-mail to PIR@csmd.edu) in addition to completing this data request form.
  • Some research projects require review by CSM’s Institutional Review Board (IRB), and that review is a separate process from submitting this data request form to PIR. Visit CSM's IRB website for more information about the IRB process.
  • The data and information produced for your data request may be shared with others outside of your department.
  • Questions marked with an asterisk (*) require an answer.
