Introduction

Premier Community HealthCare Group, Inc., (PCHG or Premier) is a non-profit Federally Qualified Health Center whose mission is to provide accessible healthcare services for all. 

Premier has begun to conduct an organizational assessment to establish an Equity, Diversity & Inclusion (EDI) program. The project seeks to include a multi-year, multi-faceted, comprehensive effort to engage employees, patients, and the local community. To that end, Premier has contracted with Estrategia Group to support them in this journey.

Purpose

This survey is an invitation to inform the Equity, Diversity, and Inclusion (EDI) journey at Premier. We will use your feedback to inform a future EDI strategy at Premier and the key informants' interviews, where we hope to go deeper in listening to your experience and insights. 

Our goal is two-fold.  With your input, we aim to create a framework emphasizing Equity, Diversity, and Inclusion to be used as a foundation on which to build upon.  Secondly, once the framework is established and agreed upon, our goal is to co-create with Premier an ED&I approach that will reflect everyone you serve and promote a sense of belonging and community.

At any stage of the journey, we invite you to connect with Estrategia Group (jessica@estrategiagroup) to share your experience, discoveries, and insights. All of the information you share with Estrategia will be kept STRICTLY CONFIDENTIAL AND USED FOR THE STATED PURPOSE.

In this survey, you will be asked to answer a series of questions. Please be honest. Estrategia Group will review these questions, and your identity will be fully protected (should you choose to share it with us for follow up). The questions will seek to explore two major themes: 

- Belonging: relates to the sense of connection, staff awareness, and inclusion practice;  growth in the community—the Spirit of Equity, Diversity, and Inclusion.

- Sustaining: relates to the operations, administration, direction, and management of the policies & procedures; culture of the organization—organizational Support for Equity, Diversity, and Inclusion.
