Perinatal Equity and Care for Everyone (P.E.A.C.E.) Project Interest Form

The Perinatal Equity and Care for Everyone (P.E.A.C.E) Project provides clients, families, & professionals with information about prenatal, postpartum, and infant resources available in Howard County. The Perinatal Care Navigator will assist clients, families, or providers in determining which services best meet the client’s needs & interests. The Perinatal Care Navigator will then link the client to community resources/services & will follow-up on the outcome of the referral.

Once the form is submitted, someone from the P.E.A.C.E Project will contact you within 24-48 hours. Please submit one form per individual and note that agency hours are 8 am - 4 pm, Monday through Friday. Additionally, we observe state holidays.
