Student Midwife Application |
When you serve our clients, you are the face of our company. As such, when you are serving our clients, we require that our assistants, employees, students and apprentices dress modestly, refrain from swearing, do not drink alcohol, smoke, or use street drugs.
You will be expected to provide your own food and transportation. It can be difficult being on call 24-7, being up at all hours of the night, being gone from home for days at a time. You need to be ready to leave for a birth at any time of the day in just a few minutes. Any time off will need prior approval with appropriate back up arranged.
Keep in mind that all client information and midwife related issues are confidential. Sharing personal information without prior permission is considered a breach of confidentiality and will be just cause for release from the apprenticeship. After your application is reviewed, you may be asked to come for a personal interview. If you are approved for an apprenticeship, we will give you a copy of additional guidelines and an apprenticeship agreement.
If you are accepted into an apprenticeship with us, you will be required to pay a $200 fee to cover the cost of your background check, annual HIPAA/OSHA trainings, and other administrative costs associated with your apprenticeship with us. Of this $200, we require $50 to be paid prior to proceeding with the background check. The balance is due before beginning your apprenticeship. Student midwives of color may be eligible for a scholarship in order to cover these costs.
Once your background check is complete, you will be required to undergo a physical exam, drug test, and TB testing. The physical exam and testing will be obtained at your own cost. You also will need to provide proof of immunizations (MMR, TDap, Hepatitis B, and annual flu shot) or signed declination.
Thank you for your sincere reflection and response to these questions. We hope it will help you in your journey to becoming a midwife. Don’t hesitate to call or email if you have any questions.
