Operation PAR Alumni Survey

We want to know more about what our current and former clients go on to do after they leave Operation PAR’s treatment programs. We also want to get better at keeping in touch with our alumni, as well as helping our alumni to keep in touch with each other.

In order to do this, we are currently in the process of developing an alumni group. We envision the group getting together occasionally for recreational activities sponsored by Operation PAR, interacting to provide support to each other, and acting as a resource for current PAR clients. It would help us if you could provide us with your feedback by taking a few moments to complete the following questionnaire. All questions are optional.
