Before adding your name to the wait list, ensure you have confirmed:

Permission from your supervisor to attend (if applicable).
A clinical preceptor for post-training consultation. (Note: those who record their own name as a preceptor on this form will not be eligible for the waitlist. You cannot serve as your own preceptor.)

Training dates are yet to be set. You will be contacted when a space becomes available.

ACCEPTANCE
Healthcare providers are accepted on a first come - first serve basis. You will receive a confirmation that you are on the waitlist. If you have questions or concerns, contact Health Educator, Lesley Baldry at LBaldry@cancercare.mb.ca.
