Understanding this survey

The following information that you will supply has three purposes:
1.) To avoid administrative confusion when gathering data in other tests that you will be responding to.

2.) To compare results. Example: are results different for men and women? How about by age groups? Are larger or smaller people doing better or worse with “long-term” COVID-19? What about results after an intervention with light? You get the idea.
3.) To do 1 & 2 while respecting the privacy of the information that you chose to provide. It stays in our database and isn’t shared with anyone outside of the study, nor with other participants.
