Question Title

* 1. Study Purpose
The purpose of this research is to investigate current knowledge and opinions of occupational therapy practitioners in the United States.

Activity Description and Time Commitment
If you agree to participate in this study, you will complete the following questionnaire (also referred to as a survey). Participation will take approximately 15 minutes. The survey includes questions about your knowledge, opinions, and demographics as an occupational therapy practitioner. A majority of the questions are designed to gather specific information where one option will be selected. However, there will also be opportunities for respondents to provide open-ended responses to allow for additional insights and perspectives. Questions contained in this survey may be skipped if you choose to do so.

Minimum Age to Participate
To participate, you must be at least 18 years of age. If you reside in the state of Nebraska, you must be at least 19 years of age.

Participation is Voluntary
Your participation is voluntary. There is no penalty for choosing not to participate or for withdrawing your participation at any time. If you wish to withdraw your participation, you may do so by exiting out of the survey window. Any questions answered prior to withdrawal will be recorded as a response. Survey questions may be skipped if you choose to do so.

Risks and Benefits
Potential risks of this study are feeling mild emotional discomfort when answering questions that some individuals may feel are sensitive topics.

Potential benefits of this study include your contribution to contemporary occupational therapy research and a doctoral student’s educational experience. By participating, your personal knowledge and opinions will be part of a data set that will directly lead to a better understanding of the current occupational therapy practitioner’s knowledge and general opinions.

As an occupational therapy doctorate student, this pilot study is meant to be the first step in a series of educational and research opportunities. Dissemination of the results of this research will lead to opportunities for skill building and networking within our profession. Further, the analysis of this data will assist in the completion of my doctoral capstone experience and therefore the completion of the graduation requirements of my entry-level doctorate program.

Privacy/Confidentiality/Data Security
We will take all reasonable steps to protect your identity. Collected data will be anonymous, as researchers will not know who participated nor which responses belong to any individual. Further, SurveyMonkey will not store IP address information.

Deidentified data from this study may be shared with the research community at large. We will remove any personal information that could identify you before data may be shared with other researchers to ensure that no one will be able to identify you from the information we share. Despite these measures, we cannot guarantee the confidentiality of your personal data.
