Behavioral Health Needs Assessment: Community Conversations Survey

Health Management Associates (HMA) is working with the Colorado Department of Human Services, Office of Behavioral Health (OBH) to conduct a statewide needs assessment and strategic plan. The goal of the needs assessment is to learn about the substance use and/or mental health needs across the state, identify gaps in services, and look at health disparities in substance use and/or mental health services. The assessment will take into consideration both the perspective of those who provide substance use and/or mental health services as well as those who are in need of these services.

We invite you to share your thoughts and experiences in this survey so we can better understand the strengths and concerns about the substance use and/or mental health system of care in your community. Information from the survey will be reported in a needs assessment report to OBH. The needs assessment will then inform OBH’s five-year strategic plan.

All answers are anonymous. Individual answers will not be released, nor will they be shared with the State or anyone else. We will not be able to identify the people who took the survey.

There are no right or wrong answers; it’s your opinion that matters!
