This Community Health Needs Assessment (CHNA) is being conducted as a collaborative effort between Androscoggin Valley Hospital, Upper Connecticut Valley Hospital, Weeks Medical Center, North Country Home Health & Hospice Agency, Coös County Family Health, and Ovation Healthcare. Your responses will help inform the top health priorities in your community.

This survey will ask you to complete the following:
  • Rank the health needs of your community
  • Provide your opinion of the last CHNA performed in 2022
  • Complete questions related to access to services in your community
