2024 PROVIDER SATISFACTION SURVEY
Dear Provider,
Community Behavioral Health (CBH) is requesting your feedback regarding the interactions we have had with your agency over the past year. Our Provider Satisfaction Survey is one of the primary means by which we gauge our performance, and your feedback is important to us. The survey will be open until February 10, 2025.
The survey begins with questions that focus on your satisfaction with CBH as a whole and then progresses to more specific, departmental questions. Responses are anonymous. Please answer each question based on your experience in 2024 and the survey will allow respondents to skip over sections that are not relevant to your experience with CBH.

We would like each provider or organization to complete as many relevant sections as possible:
- Member Services
- Provider Operations (Provider Relations, Provider Training & Development, Provider Contracting)
- Clinical Utilization Management
- Claims Management
- Quality Management (including Pay for Performance)
- Program Integrity (Compliance)
- Credentialing & Re-Credentialing
- NIAC
All responses will be reviewed confidentially and the final results will be posted on the CBH website as an annual report.
