The Colorado Department of Public Health and Environment, Health Facilities and Emergency Medical Services Division wishes to thank you for your participation in the following survey.

The purpose of this survey is to gather information from small capacity residential assisted living facilities, 19 beds and fewer, to better understand how specific areas of the new licensing requirements would be or are applied in small assisted living residences. The information gathered from this survey will be used by a workgroup which is comprised of current ALR providers, to determine where areas of guidance, technical assistance and/or provider training may be of immediate value. This information will also be used to determine if amendments to Chapter 7 Assisted Living Residence licensure rules are necessary for small capacity assisted living residences.
