This online form is utilised for Affiliates to be able to submit uniform designs for NSWTA representative events in accordance with the Section 4.1.1 on Page 23 of the 2018-19 NSWTA General Conditions of Entry.
Submissions are to be made via the nominated Senior or Junior Affiliate Coordinator for the particular event.
Please upload all designs of club uniform into categories provided below. The NSWTA reserves the right to reject designs. If a design is rejected the Affiliate will need to submit a new design.

2018 Country Championships Submissions for the 2018 Country Championships are required AT LATEST by 5pm Friday 21st September 2018.
2018 State Cup Submissions for the 2018 Country Championships are required AT LATEST by 5pm Wednesday 7th November 2018.
