Introduction

Who's asking?

Montezuma-Cortez District Accountability Committee (DAC), a parent led committee.

Why are we asking?

We would like to help make the district and schools more welcoming to parents, and help parents be more involved in their child's education.
What will we do with your answers?

We will make recommendations to the school board and the district administration based on your answers. We will also ask the district to share the results with all district families. We may repeat the survey next year to determine if the district is making progress.

Please fill out as honestly as you can. Your answers will be anonymous unless you choose to enter your contact information at the end. You may fill out the survey multiple times if you have multiple children enrolled.
