Introduction

The Nebraska 2017-2021 State Health Improvement Plan (SHIP)- Depression and Suicide Priority, is looking to highlight agencies that promote social, emotional well-being among Nebraskans. Sharing stories about successful programs and services that strive to prevent or decrease depression and suicide rates, will increase awareness about the breadth of promotion, prevention and intervention activities statewide. The contribution of this important work is honorable but often under recognized. SHIP seeks to shine the light on successful programs that have positively impacted individuals, families and communities in Nebraska.

The purpose of this survey is to: 1) Recognize agencies across Nebraska that provide some type of program/service related to social-emotional-mental well being, including efforts to prevent or decrease rates of depression and/or suicide; and 2) Gather success stories from agencies about successful programs or services.

This survey should take about 5-10 minutes to complete. This is not intended to produce a comprehensive list of all agencies, programs or services. Our hope is to broaden the public's understanding of how promotion, prevention, and intervention strategies collectively contribute to a holistic approach to addressing depression and suicide. Your stories will be accumulated into a report and distributed to partners statewide. For clarity, a disclaimer will note that your agency and program or service may not be directly related to the SHIP, but is shared as a reflection of a broad network of organization that care for the human condition.
