The BTG program at Prisma contains the 40-Hr minimum basic training to become professionally trained and qualified, providing proof of successful completion for work eligibility in healthcare settings, community interpretation, and as 1/5 requirements to sit for the National Certifications through NBCMI (CMI, Hub-CMI) or CCHI (CHI, CoreCHI).

I hereby release from liability Prisma International Corporation and its representatives for seeking, gathering, and using such information to make admission decisions and all other persons or organizations for providing such information. I understand Prisma International Corporation uses vendors to provide some of its services and cannot be held liable for any reason outside of their control, including, but not limited to, 3rd party services, products, systems, venue rentals, valet parking services, publishers, restaurants, catering services, internet providers, transportation, and parking partners, etc.

I hereby understand that Prisma International Corporation shall not be held liable for any type of liability, including, but not limited to, the student’s behavior while on site, damage or loss of the student’s property, liability within the company premises including, but not limited to, the company’s building, parking space, and surrounding areas.

I understand that any misrepresentation or material omission made by me on this application for admission will be sufficient cause for cancellation of this application, or immediate termination of services rendered, whenever it may be discovered.

I acknowledge I will not hold Prisma International Corporation liable for any mailing delays caused by unforeseen reasons, force majeure, or lost mail by DHL, USPS, UPS, FedEx or any other mail delivery service provider; where a period of 15-60 days may apply for a lost-mail claim to be processed. Prisma International Corporation requires a delivery signature for their packages.

I acknowledge that there is a specified length and time for services, and that this application does constitute an agreement or contract for services until payment is received/unless payment has been received. Accordingly, either I or Prisma International Corporation can terminate the relationship at will, with or without cause, at any time before Prisma International Corporation confirms my class registration and seat booked in writing; as there is no violation of applicable federal or state law.

I acknowledge and understand that I will cancel my registration if no payments have been disbursed to Prisma International Corporation. I also acknowledge that I understand if costs are incurred by Prisma International Corporation for booking hotel/accommodation/conference room for this class that requires a minimum of participants to hold the class; that my registration fees are forfeit if I am unable to attend the class for which I register (example: signing for a conference you cannot attend anymore).

I acknowledge and understand that the payments are due four weeks prior to training and up to 10 calendar days before the first day of class, to guarantee my books will be shipped on time for my class.

I acknowledge and understand that if Prisma International Corporation ships the books to the address I provided on this registration and I move, but fail to inform them about the new mailing address where the books or certificates of completion must be shipped to, and this action results in the mail carrier returning the books or certificates to the sender (Prisma International Corporation), that the company will issue you a separate bill to ship the books or certificates again; inclusive of handling fees, shipping costs, shipping supplies incurred by the company or by any of its approved partners (Carriers). Alternatively, if located near our GA site, you are welcome to pick up your certificate personally after setting up an appointment.

PLEASE NOTE:
No refunds are available unless you cancel your payment or request a partial or full refund within 3 calendar days from purchase. Note that you, as the consumer, will be charged with all transaction fees incurred by Square, PayPal, or any other payment processing vendor. Your refund will be deducted for credit card processing, shipping, and handling.

If you cannot make the class, your class registration fees may be applied for the next classes available within the same calendar year on your registration form; only the following cases apply: medical emergency, which requires a note from a medical provider or school/workplace, loss of an immediate family member, sudden relocation, or job loss.

If you are late for class (up to 10 minutes for any given session), you will be responsible for the lesson/partial lessons you have missed and must study on your own. You can purchase additional make-up sessions at a cost. We highlight instructions once you are registered to ensure a prompt start time, so we do not inconvenience other students. Prisma International Corporation reserves the right to remove you from the class if you have missed more than 15 min for each of the sections (8 sections: morning and afternoon for 4 dates of class).

If you do not show for your post-class coaching session (1), you will forfeit the remaining session(s) included in your package, unless you contact the Coach / Trainer at least 24 hours prior for rescheduling. You will also forfeit any scheduled session after 10 minutes tardiness, when the Coach has been scheduled.

Coaching is a value-added service as it is not guaranteed to be provided after training has been rendered, as this is a pro-bono portion added to your package for candidates who successfully pass their exam at 70% or higher.

All sales are final. If the minimum cohort or class size does not form, you have the option to take the online options of the course; if not, you will be immediately enrolled in the next available on-site training if applicable. AMEX, VISA, DISCOVER, MASTERCARD CONSUMERS' NOTICE:
For credit card payments, if there is a filed dispute to get a partial or full refund, you as the consumer will be charged all processing fees and costs associated with the investigation including, but not limited to, accounts receivable specialist hours consumed/spent to conduct the investigation, cost of capital from money withdrawn while the dispute is investigated (prorated to % rate of days in the investigation of the dispute filed, the time spent report to the processing/banking entity all documentation to support we deliver and/or planned to deliver and that you had agreed to the terms, prior to filing a dispute; all legal fees associated with mediation, arbitration, lawsuits, attorneys fees, court costs in the state of North Carolina, should the dispute result of you, as the consumer, to be at a loss. Note, on the other hand, that if you, the consumer/buyer, wins the dispute filed, then a partial refund may be available after all transaction fees incurred by Square, PayPal, or any other payment processing vendor have been deducted. Your refund will be deducted for credit card processing, shipping, and handling.

MEDIATION, ARBITRATION and LEGAL CLAUSE:
In the case of a legal dispute regarding this enrollment agreement to hold a seat for the selected class you registered, and only when a payment has already been submitted, and after the 3-day calendar days grace period for you to withdraw you application or request your partial refund, all legal fees associated with representation for Prisma International Corporation for mediation, arbitration, lawsuits representation, attorneys fees, court costs and admin fees associated with the dispute will be resolved in the state of North Carolina and will be the fiscal responsibility of the losing party.
