NCM next steps for "Building Capacity for Comprehensive Primary Care" |
The Care Transformation Collaborative of RI/PCMH Kids Board of Directors has started working on our next strategic plan and is considering “next steps” in our work to build capacity for comprehensive primary care.
The National Academy of Sciences (2021) provides us with an updated definition of high quality primary care:
It is the provision of whole-person, integrated, accessible, and equitable health care by the interprofessional team who are accountable for addressing the majority of an individual’s health and wellness across settings and through sustained relationships with patients, families and communities.
The Care Transformation Collaborative of Rhode Island/PCMH Kids uses our best practice sharing committee meetings, learning collaboratives, NCM/CC core curriculum training program, and Annual Conference, to create a learning community for our primary care practices, to assist you in your efforts to effectively deliver integrated whole-person care for people living in our communities.
With this in mind, we seek your input, looking at how we can best support your efforts on the care team with implementing high quality primacy care and in reaching out into and partnering with the community to address issues of access, health equity and social determinants of health.
Please complete this survey by noon, June 11th. We will bring your input and suggestions to the Nurse Care Manager/Care Coordinator Best Practice Sharing Committee meeting on June 15 and share this discussion with the CTC-RI Board of Directors. Thank you!
