Community Suicide Prevention, Mental Health and Resilience Needs and Strengths Assessment

The following questions are designed to help the North Carolina Home Builders Association develop a comprehensive and sustained suicide prevention, mental health promotion and well-being program.

Confidentiality: Responses will be kept private and confidential to the extent permitted by law. In any sort of report we might publish to the North Carolina Home Builders Association, we will not include any information that will make it possible to identify a survey participant. All survey responses are anonymous and not linked to any individual respondent and stored securely only on password protected computers of the project team leaders. 

Possible risks to participants: The risks of participating in this survey are minimal. This survey includes questions about distressing professional or personal experiences with behavioral health and/or suicide, and some people may experience anxiety or become emotionally upset while answering these questions.

If at any time you would like to speak with someone confidentially regarding distress you might be experiencing or if you have concerns about others, you can contact:

Possible benefits to participants: Although there are no direct benefits anticipated from your participation in this survey, your contribution may help the effectiveness of the future efforts at the North Carolina Home Builders Association to develop a suicide prevention, mental health promotion and well-being program. You may feel positively about this contribution.

Compensation: There is no compensation for this survey.
