Introduction
What is the purpose of this survey?
The Amyloidosis Research Consortium (ARC) is conducting this online survey to understand the experiences and needs of amyloidosis patients and their caregivers.
What am I asked to do?
This online survey will take about 30 minutes to complete. Participation is voluntary and you can leave the survey at any time. Responses gained through this survey will be confidential. Patient privacy is secured because all responses are de-identified and caregiver comments cannot be traced back to the patient.

Will I be paid for taking part in this research?
You will not be paid for taking part in this research. However, U.S. residents who complete the survey can enter a drawing for a chance to win a $50 gift card as a thank you for your participation. To be entered, simply provide your name and email after completing the survey.
