After-School Program Planning Survey for Families

Purpose of the survey

This survey is for parents/guardians of Conwell Elementary School's remote and in-person students who are currently in grades preschool through grade 5. Please complete this survey if you are interested in having your child/children attend this year's summer program and/or next year's school year program. 
 
The after-school program is applying for the DESE 21st Century Community Learning Center Grant. If we are awarded this grant it would begin this summer and would be available for the next 5 years. The goal of the grant is to provide interactive, relevant, and engaging programming during the school year and summer that meets the specific academic, social emotional learning, and developmental needs of students. To help us in our planning we are asking for parent/guardian input. 

To help you in answering the following questions, the after-school program is planning the following:
  • The summer and school year program will follow all school district COVID policies and procedures. 
  • If safe to do so and within our COVID guidelines, all classroom cohorts will be able to participate in the school year program, Monday through Friday.
  • We are planning on our 2021 summer program to start August 2nd and will end August 13th.  (If we receive this grant the summer program may be extended).
  • With grant funding the program will be free to Conwell students.

3.How many days a week would you most likely register your child/children for the school year program? The grant's goal is for students to participate in the program at least 2 days a week (ideally everyday of the week).

6.This year's summer program will start tentatively on  August 2nd and will run 2-3 weeks. If you are interested in having your child/children attend a summer camp program what hours work best for your family? The grant's goal is for students to attend the entire program day.
