The survey you are about  to complete is part of an annual assessment your child’s school uses to seek-out and understand the thoughts and experiences of the school community (students, parents, and the adults who work at the school). As a parent or guardian of one or more students at this school, your feedback will be used to make choices, set goals, for continued growth. 

WHAT WE NEED FROM YOU: As you respond to each item, focus on you and your student’s experience with the school over the past year (or since starting at this school).
