1. Welcome to SANAC survey on the impact of COVID-19 on CBO service delivery

November 24, 2020

Dear Participant:

My name is Ozius Dewa and I am a lead consultant at Tinogona (“Reakgona”), a management consulting firm contracted by the South African National AIDS Council (SANAC) to conduct a survey on HIV and TB community-led response in the time of COVID-19 pandemic. The purpose of the survey is to determine the HIV, TB and STIs service delivery disruptions encountered by community led and community-based organisations in the context of COVID-19, including eliciting advocacy measures in place. The findings of the survey report will inform SANAC on the critical role and impact of HIV community-led responses as well as the current challenges and opportunities they face due to COVID-19.

The following questionnaire will require approximately 30 minutes to complete with most questions requiring you to tick applicable options. There is no compensation for responding nor is there any known risk. In order to ensure that all information will remain confidential, the insertion of your name has been made optional. If you choose to participate in this project, please answer all questions as honestly as possible and return the completed questionnaires promptly. Participation is strictly voluntary, and you may refuse to participate at any time.

Thank you for taking the time to assist SANAC in understanding the operational environment of community-led organisations and their network in the context of COVID-19. Completion and return of the questionnaire will indicate your willingness to participate in this study. If you require additional information or have questions, please contact me at the number and email listed below.

If you are not satisfied with the way this survey is being conducted, you may report any complaints to Lifutso Motsieloa, the SANAC Technical Lead: Monitoring and Evaluation; Email: Lifutso@sanac.org.za  Phone: 0127481007

Sincerely,

Ozius Dewa
