This form is only for people who wish to receive CEs on their certificate. DO NOT fill this form out unless you are ready to provide an ASWB, NBCC, NAADAC, or MNA license number on this form.

Everyone who attended the training will receive an automatic certificate sent to their email. If you wish to receive CEs along with your certificate of completion, please fill out the form below.

**Please note: You will only be able to get a certificate for our online training if you were signed into Zoom during the entire online training.

If you are requesting a certificate more than two weeks after the training, please email praxis@c4innovates.com directly so we can process your request.
