About this survey:

1) Purpose: To collect information and opinions from Medway residents, employees, and business owners about the topic of Diversity, Equity, and Inclusion

2) Length of Survey: It should take approximately 10 minutes to complete

3) Your Data: Your responses to this survey will remain 100% anonymous and will not be used to identify individuals for any reason. Should you wish to submit additional feedback to the IDEA Committee, you can do so using this form (here)
