Informed Consent Notice - Collecting the information in the following survey is for the sole purpose of knowing how satisfied students' staff and faculty are with the Meal Plan and the food service provided to them. This survey DOES NOT collect any names, emails or student ID numbers. By accepting below, I consent to allow Dana Hospitality to collect my personal opinions and information on this survey using SurveyMonkey. The data will be stored on SurveyMonkey’s Canadian servers; however, your data will still be accessible by SurveyMonkey’s teams in the United States, Ireland, and Australia for the purpose of implementing, maintaining, repairing, troubleshooting or upgrading services provided by SurveyMonkey. All responses will be removed from the SurveyMonkey platform on or before January 30, 2025. Dana Hospitality will use and retain aggregate reports extracted from SurveyMonkey for the purpose of reviewing and improving the Food Services.
