Welcome to the Macon County Mental Health Board’s Unmet Needs Survey!

This survey has been designed to collect responses from those working and residing in Macon County regarding unmet needs related to three major disability groups-- Mental Health/Mental Illness, Developmental Disabilities, and Substance Use. Responses from the survey will be used to compile the Unmet Needs Document that will provide a comprehensive overview of the unmet mental health service needs in Macon County. The identification of these unmet needs will be used by the Macon County Mental Health Board when allocating funds to new programs as well as social service agencies for program development and grant funding opportunities. This survey is part of the Macon County Mental Health Board's One and Three Year Planning Process.

Respondents may complete the survey for one disability group or multiple disability groups.
