Potential Candidate Form
Thank you for your interest in identifying a potential candidate (either yourself or someone from your state) for the Milbank Fellows Program. The strength and success of the program starts with interest from state leaders like you and we greatly appreciate your help identifying potential candidates. Please take about 10 minutes to fill out the following form. You may close out and return to the form if on the same browser and computer, but we recommend filling it all out at once, including uploading your resume and headshot.
The window for nominations begins May 1st, with interviews to be scheduled through June on a rolling basis. Applications will be accepted until June 15th. Final selected candidates will be notified by late July.

About the Milbank Fellows Program
The Milbank Memorial Fund believes that state leaders are the drivers of innovation and improvement in population health. We are committed to equipping and supporting leaders in the executive and legislative branches of state government with the knowledge, leadership capacities, and connections to navigate the complexities of systems and health policy to advance population health. The Milbank Fellows Program (MFP) is our newest offering to support state health leaders.
Through this program fellows will:
• Gain new knowledge about specific population health issues, interdisciplinary health and human services systems, and evidence-based approaches to improving population health;
• Expand their personal insight and leadership capacities in support of defining and advancing a population health vision and agenda;
• Create meaningful connections with a network of colleagues, both from within the cohort and the Milbank State Leadership Network, so they can leverage their new knowledge and capacities to take action in their state.
Fellow Selection Process
The Fund will select up to 24 participants for the 2024-2025 Milbank Fellows Program cohort. To express interest in the program, please complete this form. If our selection team determines you would be good fit for the program, we will ask for a resume and an interview.
