Thank you for your willingness to take Muslim Community and Health Center's (MCHC) survey.
The purpose of this survey is to understand the needs of the many communities that MCHC serves. Whether or not you are a MCHC patient, we are interested in learning about you, your family, and your community so that we can better meet your needs and the needs of those around you.
The survey is voluntary, anonymous, and confidential. We will not ask for your name and all the results will be looked at together and reported on as a whole. If you do not wish to answer a question for any reason, you may select “I choose not to answer”
The survey is expected to take approximately 10-20 minutes to complete. If you close the survey, the computer will NOT remember where you are so it is important that you complete the survey in one sitting. If you leave the survey and wish to complete it, you will have to start from the beginning.
The benefits of participating in this survey are that you will be helping MCHC understand perspectives of community members so that it can improve its services.
