2024 Excellence in LTC Nomination Form |
Nominator Information
Across Maine, dedicated healthcare workers provide vital care, supportive services, comfort and peace of mind to some of our most vulnerable citizens. Every year, in partnership with the Maine Long Term Care Ombudsman Program, MHCA looks forward to celebrating the best and brightest of these individuals at our Excellence in Long Term Care Awards.
From home care to assisted living to nursing facility care, we know that caregivers are the backbone and lifeline for Maine’s older and disabled citizens. It is our pleasure to honor their dedication, performance and all the exemplary things caregivers do to provide high quality care, reassure loved ones, and truly make a difference in the lives of their residents/consumers.

Because many team members contribute to the quality of care and life, we present caregiving awards across all disciplines as follows:
1) Nursing/direct caregiver, such as a Nurse, CNA, PSS, CRMA, HHA, DSP, or other direct caregiver, AND
2) All other disciplines, like social work, dietary, activities, etc., from any other department
Facilities and organizations may submit one nomination in each category for a total of two nominations. All nominees must be employed by their organization/facility for at least one year. An independent panel of judges will select the winners, who will be recognized during a special live event on November 14.
Thank you for taking the time to recognize an employee's dedication to Excellence in Long Term Care.
