Thank you for your interest in the SHM MARQUIS Med Rec Collaborative program. This survey serves as your application for participation in the program. Applications will be reviewed on a rolling basis. The next cohort will begin in February 2018.
This application is to make sure that participating sites have the commitment and ability to implement the program and make iterative refinements throughout. The MARQUIS Med Rec Collaborative starts with a two-month introductory phase to help get acquainted and up and running with the program followed by a 12-month Collaborative Program.
This program is only open to adult hospitals, and there is a fee of $4,700 to participate in the program.
