SURVEY INTRODUCTION

The Oregon ACP is inviting you to participate in a WELLNESS SURVEY-2024.
This survey will ask questions about your practice, your stress, and your emotional wellbeing. The Oregon ACP hopes to use survey results to more fully understand the needs of Oregon ACP Chapter members.
This survey could be distressing to some. If you need help or support, resources are listed in our email message.
Protecting your privacy is important to us, and data will be summarized so that no individual can be identified by their responses. We are aiming to have summary data available in spring 2024.
Your participation in this survey is completely voluntary and you are free to choose not to participate for any reason.
The survey will take approximately 5 to 10 minutes to complete.
