With the Health and Social Care Secretary's recent decision to scrap dementia diagnosis rate targets from the NHS Operational Guidance 2025-26, it is clear the dementia is not one of the government's priorities. With nearly a million people in the UK living with dementia, where a third remain undiagnosed, the voices of these individuals have been muted. We want to hear about your experiences with dementia care and support.

We are working alongside partners to develop a report that highlights the experiences of people living with dementia. This report will aim to improve dementia care and support across the sector. Your thoughts, perspectives, and experiences are essential in the development of this report to ensure we are advocating effectively to address the needs of people living with dementia.

This survey is designed to be simple and easy to complete. There are no right or wrong answers—just your honest opinions and experiences. If you need help filling it out, a family member, friend, or caregiver can assist you.

By sharing your views, you will help us understand what works well and what can be improved for people living with dementia.

Thank you for your time and for sharing your experiences with us.
