Proposed Framework for 2020
Full Cohort meetings - We are proposing full Cohort meetings approx. once per quarter (rather than monthly), bringing in topics of relevance across providers and facilitating shared learning and networking. We will tie equity into each full Collaborative meeting – e.g. identifying the gaps related to the topic, best practices, etc. In 2020, the Learning Cohort will include our January & August Cohort contracted clinical partners, as well as contracted SDOH partners.

Equity trainings - We anticipate hosting a few larger trainings on equity. Based on the results of the Equity Assessment in October 2019, we will determine highest impact trainings for the region as a whole. 

Topical trainings - We will have optional, more topic/sector focused trainings 1-2 times per quarter. This may include bringing in outside trainers, Lunch & Learns, and webinars. We will capitalize on local expertise and already-funded training (such as those offered by DOH).

Technical Assistance & Contract Workshops - We will continue to provide individual/small group TA as requested by providers through our TA bank. BHT will also do workshops for Year 2 Contracts for our January & August Cohort members, as a best practice from Year 1.

* 7. We are revamping our TA Bank in 2020 to make it more useful to you. This is a "bank" of consultants that partner organizations can access for free one-on-one technical assistance (TA), for example, in developing a workflow or registry, implementing equity strategies, building a care team, launching telehealth, etc.
