Lakeview Center Community Survey 2024-25 |
Introduction
Lakeview Center, Inc. (LCI) is assessing healthcare needs and services in the local community. LCI would like to hear from you if you are aged 18 or older and:
- a person who receives or has received services from LCI,
- a family member of a person who receives or has received services from LCI,
- a healthcare or behavioral healthcare service provider in Escambia, Okaloosa, Santa Rosa, or Walton County, Florida, or
- a resident of Escambia, Okaloosa, Santa Rosa, or Walton County, Florida.
This survey asks a little bit about you, like your age, race, and county of residence, as well as your experience receiving several types of healthcare services, but it will not ask for your name or any other information that could identify you. At the end of the survey, you will be offered the opportunity to enter a drawing for one of 20 gift cards (valued at $25 each) by following a link to a completely separate location to enter your name and contact information. Your survey responses will never be connected to your name.
Completion of the survey should not take more than 15 minutes. Your participation is completely voluntary. You may freely choose to decline to participate, skip any questions you prefer not to answer, or discontinue your participation at any time without any penalty to you. The information obtained from this survey will be used to assess the healthcare service needs of our community, inform plans to enhance healthcare services in our community, and monitor progress over time.
