The purpose of this form is to evaluate performance and to serve as tool to develop and promote USS officials
DIRECTIONS:
Officials - Please complete one evaluation for each official officiating in your discipline. If applicable, indicate whether you are the assigned chief or assisting at the competition. 
Chief Referee - Please complete one evaluation for each referee on your team and for each chief official assigned to the competition.
