LTC Model Policy for Testing Feedback

ISMP Canada is pleased to provide several LTC Medication Management Model Policies for testing in your Home. We ask that you to complete the following questionnaire once you have adapted and implemented the Model Policy. Your feedback will be used to develop a finalized Model Policy that can be used by LTC Homes in Ontario to drive medication safety.
