The Institute for Safe Medication Practices Canada (ISMP Canada) is a national, not for profit organization committed to the advancement of medication safety in all healthcare settings. We are working with the Ontario Ministry of Long-Term Care (LTC) on a three year initiative called Strengthening Medication Safety in LTC. Resident and family engagement is a key focus are for sustaining quality improvement and the toolkit is one way to support staff. The feedback received from you will help us to continue to provide you with meaningful resources.
