The Transition Academy Member Registration Form

Transitioning from high school to college or a job can be overwhelming, but you are not alone. The Transition Academy (TTA) is a free support service for youth with Developmental Disabilities in the Kansas City area, and has an Individualized Education Plan (IEP). The Transition Academy is here to help you and your student to navigate resources and create your individualized path to success.

Please provide complete and accurate information. This information will help The Transition Academy provide you and your student with free resources and information while receiving services through The Transition Academy, including admission to the KC DiversAbility College and Career Fair, 1:1 social services support, and referral to our KC DiversAbility Collective network.
