Stories in Power Survey |
Introduction
The Stories in Power Project is a collaboration between the Health Forward Foundation and a local consultation firm, Ad Astra Community Innovations Group. We hope to gather information about community members' experience with power or the lack thereof in decisions that affect their lives.
This survey consists of several questions that will ask you about your experience with power or your community's experience with power. For the purpose of this survey, we ask that when you think about your community you consider the community in which you reside (such as your neighborhood) or a community to which you belong because of a shared interest or shared experience (such as having experienced houselessness or belonging to a specific religious group). The survey takes about 10 minutes to complete. Answers are completely anonymous. If you have questions about the survey, please feel free to email admin@adastracommunityinnovations.onmicrosoft.com.
